Questions to ask your care team
- What are the main goals for my baby today?
- Which changes should we tell the team about right away?
- Can you explain this equipment or monitor in plain language?
- What needs to happen before the next step in care?
- Who should we contact if we have questions after leaving the hospital?
These are general prompts, not a checklist for deciding whether a baby is ready for discharge.
Preparing for a transition home
Every baby's plan is different. Ask the clinical team to explain medications, feeding plans, follow-up visits, equipment, warning signs, and whom to call with concerns.
Do not use this website to decide whether your baby is ready to go home. That decision belongs with the baby's qualified care team.
Support for caregivers
It can be hard to take in information during stressful moments. Consider asking a trusted person to take notes, requesting an interpreter if needed, or asking the team to repeat information.
If you feel overwhelmed, ask the hospital team whether a social worker, family-support specialist, or mental-health professional is available.
Family support organizations
